Home-Aide Care Solutions also provides intensified case management at an additional cost. Our registered nurse staff can assist with organizing the multiple disciplines of care that occur in the home. They will communicate with your primary care physician, physical therapy, occupational therapy, hospice team, and any other disciplines to coordinate care. Our nurses can help set up appointments, find necessary equipment, and supplies. We are able to provide whatever information and support is needed to create safe and successful individualized care for you or your loved one.
